Day Services Referrals
This page is dedicated to referring individuals for Day Services. Our welcoming and engaging Day Program provides the opportunity to build friendships, discover new interests, participate in meaningful activities. Designed to support individuals with a wide range of abilities, support needs, and personal goals.
We are currently accepting new participants and would love to connect with individuals, families, guardians, case managers, and referral partners who are looking for a supportive place where people can feel valued, build confidence, and enjoy enriching experiences.
Day Service Referral Form
Once your referral form has been received, our team will carefully review the information provided and reach out to discuss next steps. We may contact you for additional details, answer any questions, and determine the most appropriate services and supports based on the individual's needs. We appreciate the opportunity to learn more about the person being referred and look forward to connecting with you soon.

If you prefer to speak with someone directly or would like to learn more about our Day Program before completing the referral form, please contact Melanie Larson, Day Services Supervisor. Melanie can provide additional information about the program, discuss an individual's interests, abilities, and support needs, answer any questions you may have, and help determine whether our Day Services program would be a good fit. We welcome the opportunity to connect with individuals, families, guardians, case managers, and referral partners and look forward to sharing more about the meaningful experiences and opportunities available through our program.
